RN Case Manager

Home care agency is seeking an experienced RN Case Manager to be responsible for assessing, planning, implementing, coordinating, monitoring, and evaluating the care and services provided to clients in their homes. The RN Case Manager will serve as the primary point of contact for clients and their families, ensuring that their needs are met and that care plans are effectively executed.

ESSENTIAL JOB DUTIES

  • Conduct comprehensive assessments of clients’ health status, needs, and preferences.
  • Develop individualized care plans in collaboration with clients, families, and interdisciplinary team members.
  • Coordinate and oversee the delivery of skilled nursing care, personal care, and support services according to established care plans.
  • Provide direct nursing care as needed, including wound care, medication management, and symptom management.
  • Monitor clients’ progress and modify care plans as necessary to ensure optimal outcomes.
  • Educate clients and families about their health conditions, medications, and self-care techniques.
  • Advocate for clients’ rights and preferences, ensuring that their voices are heard and their choices respected.
  • Communicate effectively with clients, families, caregivers, physicians, and other healthcare professionals.
  • Maintain accurate and up-to-date documentation of assessments, care plans, interventions, and outcomes.

QUALIFICATIONS

Current licensure as a Registered Nurse (RN) in the state of New York.

Minimum of 5 of clinical nursing experience, preferably in home health, hospice, or a related setting. Demonstrated knowledge and proficiency in nursing assessment, care planning, and coordination of care.
